The Enrollment Workbook is not an application for the Bonner AmeriCorps Program; it is a contract. Once the Foundation receives the completed Enrollment Workbook, a student will be enrolled and committed to completing a term of AmeriCorps service.

The Foundation is required to enroll each member within 25 days of their start date. They ask, therefore, that all Enrollment Workbooks arrive at the Foundation no later than 20 days after the first day that the student desires to begin their term of service. All hours counted toward the student’s AmeriCorps commitment must be served between the official start date and the required completion date. You can review your start/end date for within BWBRS.

 

The Enrollment Workbook must include all of the following in order for a student to be enrolled:

  • A copy of Citizenship or Permanent Resident Documentation (i.e. Birth Certificate)
  • A copy of member's high school diploma or GED certificate
  • Campus Administrator’s signature after reviewing the checklist;
  • The Member Application must be filled out completely and signed by the AmeriCorps member and cannot be dated before their desired start date;
  • The Enrollment Form must be filled out completely and legibly, with special attention to the social security number, and signed by the member;
  • Carefully review the Member Contract, filling in blanks where appropriate. Please be sure to record the same desired start date in the Member Contract as well as on the Enrollment Workbook cover to avoid unnecessary confusion. The signature date cannot be before the desired start date. The Contract must be signed by the member, campus administrator, and parent if the member is under 18 years of age;
  • The member must sign the Drug-Free Workplace Policy, the Prohibited Activities list, the Informed Consent, the Criminal Background Check Authorization and National Sex Offender Registry form with a photo id (unless already conducted by the school or service site), and the Waiver of Responsibility;
  • The member should provide emergency contact information even if such information is already on file with the school and/or service site;
